Holland Bloorview Baseline Testing

Dear Parent/Guardian,

This is the registration process for your child to receive baseline concussion testing with the Holland Bloorview Concussion Centre. We are leaders in pediatric concussion and are very excited to provide the best concussion care in order to get kids back to the things they need, love, and want to do.

The baseline testing fee is $90 (+HST) for 1 individual child to complete testing with us. If you are registering more than 1 child than please choose the small group rate ticket type for the cost of $75 (+HST) per child. This ticket type would also apply if you are registering your child seperately but completing testing with a small group of teamates or friends at a pre-scheduled date and time. If you are completing testing as part of a team and have been told to register through this method than please choose the team rate ticket type of $65 (+HST).

Please note our program is only eligible for youth between the ages of 8-18 years of age.

If you are interested in your full team to enroll in our baseline testing program together the cost would be reduced to $65 (+HST) per child as the team would complete testing at the same time. Please contact James Murphy (Ext. 6490) before registering in order to set up a specific date/time to complete testing and a private link will then be sent out for your team to register.

Overall, the cost you choose includes the pre-injury baseline test and all follow-up physician-led care here at Holland Bloorview Hospital if a concussion occurs within 1 year. Their is no addtional cost for all care that is provided as it is covered by OHIP or by the cost of the baseline test. This baseline test is updated annually so that we can get new results for these measures as kids are constantly developing and changing in their cognitive and physical ability over time. We will contact you closer to your 1 year expiry date to remind you to renew your child again.

Please remember to have your child bring running/indoor shoes and comfortable clothes for testing.
